Not reproduced. Maybe you use some resident program, that install global hook to keyboard input.

No, there is no global hook to keyboard.
It happens only in AkelPad 4.9.4 and ContextMenu plugin 16.6 when you press OK or Cancel by Enter (during closing modeless dialog).
If you press Show button or press Enter in another place, it works correctly.
And in AkelPad 4.9.3 and ContextMenu plugin 16.2 all buttons and Enter are working properly.

Does Enter transfered to MS Notepad if it were open instead of Total Commander? Can you send or share your AkelPad folder?

Now I tested on raw AkelPad, only one plugin in Plugs directory - ContextMenu.dll without ContextMenu.ini (WinXP x86).
Enter key is transfered to the following windows: Total Commander, Far, Firefox, Thunderbird, Sublime Text.
Isn't transfered to: Double Commander, Free Commander, Multi Commander, Internet Explorer, MS Notepad, Notepad++, PSPad.
